Prairie View Care Center

Woonsocket, South Dakota
Prairie View Care Center (PVCC) came to SourceGroup through a referral from one of our existing assisted living and skilled care provider clients, Mission Healthcare. PVCC had a dramatic need to expand its facility as well upgrade its existing physical therapy, kitchen and laundry facilities to keep up with the growing needs and demands of the surrounding community.

SG began by conducting an in-depth multi-stage facility and service needs assessment. SG then worked with its architect to apply the study's findings to develop a building plan that would allow PVCC to upgrade building deficiencies that had been cited by the South Dakota Dept. Health, maximize its potential State and Federal reimbursements and meet the needs of the growing community.

Williams & Wimmer Dental

Edina, Minnesota
SourceGroup worked with the architect, Williams & Wimmer management and Patterson Dental to deliver this clinic build-out under an exceptionally fast schedule. The existing clinic was being razed and redeveloped into the new Crosstown medical center. SourceGroup completed construction of the new space in 65 days.

The clinic design includes four hygiene chairs and two private procedure rooms. Multiple offices for clinic staff, a staff lounge, sterilization area, lab and laundry were efficiently laid out to produce a pleasing environment for both patients and employees.

Northwest Eye Associates

Northwest Eye Associates selected SourceGroup as their contractor of record for all four of their last clinic projects. Our ability to deliver each project under demanding scheduling and budget requirements as well as work with clinic staff to put together a plan to minimize clinic service interruption in the case of the remodel and expansion projects has been a key reason for our ongoing relationship with Northwest Eye Associates.

Dassel Medical Center

Hutchinson, Minnesota
SourceGroup was selected as the developer for this clinic for Hutchinson Area Health Care (HAHC).

HAHC has operated a clinic in Dassel for many years and had been planning over the past several years to replace the existing facility to increase visibility & efficiency. Within the first 30 days of occupying their new facility, patient visits exceeded their projections by more than 50% and there were 72 first time patient visits!

This clinic houses 9 exam rooms, radiology, and a procedure room. The design includes the ability for future expansion to coincide with the expected success of this rural healthcare facility. HAHC color & design elements (Familiar Faces Opening New Doors) were incorporated into Dassel Medical for brand identity & continuity.

This family practice clinic has 3 medical lanes containing 20 exam rooms, plus an occupational therapy area and a community room.

Design incorporated need for extended-hour Urgent Care and mobile MRI access as well as easy pick up and drop off under the canopy at the main entrance.

Placed strategically in an aging area of the community, the clinic has become the core of medical activities for local residents.

Gateway Family Healthcare Clinic

Moose Lake, Minnesota
This family practice clinic has two medical lanes with 25 exam rooms, 3 procedure rooms as well as larger suites for X-Ray, Mammography, Dexascan, Echo and Ultra capabilities. A large Pharmacy & Ophthalmology exam rooms were added to bring additional revenue generating streams and provide increased value to the community.

SourceGroup conducted land searches on the client's behalf and ultimately negotiated and purchased land for the clinic on Mercy Hospital's campus to offer the community and clinic staff ease and convenience

Raiter Clinic

The Raiter Clinic had a great location but a facility that was too small & dated to meet the public's needs. Clinic staff worked with the architect & SourceGroup to design an 8,000 sq. ft. expansion & extensive remodel of the existing clinic that would meet the clinic's business objectives & make the project financially feasible.

Great care & planning was done at the on-set of the design process that allowed the clinic to operate without interruption or incident while the new construction & renovation of the existing facility took place.

St. Lucas Care Center Assisted Living

Faribault, Minnesota
Increasing competition motivated the owner to upgrade this 80-year-old facility. SourceGroup was selected to manage the extensive renovation of this 32,000 sq ft 4-story building. The modernization includes converting the current double occupancy rooms to single occupancy suites as well as upgrading many common areas throughout the building including administrative offices, main reception area/desk, activity and dining rooms. SG and the owner worked with an interior designer that specializes in assisted living and nursing care facilities to make sure the facility's finishes could compete with market demands.

SourceGroup developed and implemented a plan that minimized disruption to the residents and staff during the construction and adhered to the strict safety/ICRA standards mandated by the Department of Health.

White Community Hospital

This hospital expansion and renovation followed a two-phase process. The expansion included a new ER department, ambulatory entrance, 10+ patient rooms, decontamination room as well as much needed administrative and clinical space. These additions delivered a positive and significant impact to the business from a cost reimbursable/Critical Access Hospital designation.

The renovation included converting over 7,000 sq. ft. of existing hospital rooms and related support areas to a new nursing home wing that helped the hospital keep pace with the demands of its community.

7500 York Assisted Living

SourceGroup was retained by 7500 York to complete the renovation of their facility because of our proven track record of successfully completing major renovations in facilities that need to stay fully functional during the construction process. SG worked with the owner's architect, on-site staff and the City of Edina to develop a phasing scenario and construction schedule that minimized resident inconvenience, staff workflow patterns, maintained all life safety standards and expedited the project in the shortest time frame possible.

Evergreen Terrace Nursing Home (Memory Care Wing)

Due to increased competition in the area, Mission Healthcare selected SourceGroup to manage the renovation of their Memory Care Wing. Some highlights of the work include demolition of the existing double occupancy rooms to convert them to private & double suites as well as renovations to the main lobby, dining room, and multiple nurse's stations.

The renovation also includes updating the building's exterior and a new 2,500 sq ft scenic garden patio with activity nodes for family & guests.

Pleasant Seasons Assisted Living

Due to increased competition, Mission Healthcare selected SourceGroup to manage the renovation & expansion of their facility.

Renovating the interior with high quality finishes, two-sided fireplace with stone surround, casework & millwork create the "wow" factor needed to be successful in this market. New administration & meeting/conference space was part of the renovation make the space more enjoyable for staff and residents alike.

The addition of a sunroom with rich, warm colors, pine ceilings, & wainscoting provide an inviting, quaint gathering area for family & guests. The addition includes a covered drive-under canopy & handicap accessible entrances, greatly increasing the functionality of the new main entrance. A new patio & landscaping off the sunroom offer residents the ability to enjoy the outdoors.

DaVita Clinical Research

Minneapolis, MN
DaVita went through a competitive bidding process on the renovation of its administrative office space and ultimately selected SourceGroup as its contractor of record. Since then DaVita has elected to negotiate three other projects on a design-build basis with SourceGroup. Our work has included renovating its existing clinical research and dialysis space, constructing a new ISO Class 8 cleanroom for their pharmacy as well as a new clinical research suite being used to obtain FDA approval for a new dialysis unit that will be the first of its kind. The cleanroom is one of only two in the United States to receive approval after being audited for space, design standard operating procedures by leading pharmaceutical company GlaxoSmithKline.

This medical office building and Ambulatory Surgery Center has 4 OR's and provides clinical space for many specialty physicians ranging from dermatologists to neurosurgeons.

The facility also has an attached 2-story, 95-stall parking ramp and a detached multi-story 232-stall parking ramp.
